In 6x09 "Ab Aeterno," we learned that while there were a few initial survivors of the Black Rock, most of the prisoners in the hold were killed by a fearful Mr. Whitfield, while the crew (including Whitfield) were killed by the Smoke Monster.
Only the prisoner Ricardo (Richard Alpert) lived long enough to interact with the island.
